The Community Health Center of Yavapai (CHCY) is committed to providing a “medical home” for patients by providing continuous, coordinated primary medical care services.
CHCY focuses on caring for the whole individual and the family unit by providing accessible, affordable, comprehensive, quality, primary health and dental care. Your CHCY Team is dedicated to providing healthcare and supportive services for people in our community.
Our board certified medical staff offer a broad range of primary care services in the areas of Family Practice, Women’s Health, Pediatric and Adolescent care. In addition, the CHCY Intake staff is available to assist you and the members of the community navigate the financial assistance process and support the submission of applications for AHCCCS, and other services based on your family’s income. CHCY is committed to assisting those who are uninsured, underinsured and insured in the areas we work and live. For our patients who are underinsured and/or uninsured, inquire about our sliding fee scale program.
Our caring staff along with our healthcare providers believes in the concept that each individual, who is often at their most vulnerable point, deserves the right to quality healthcare, from the moment their appointment is made until they complete their healthcare experience. That is why nearly 11,000 patients refer to Community Health Center of Yavapai as their medical home.

The CHCY becomes a patient’s medical home when the CHCY is established as their source of primary medical care services. The patient is then referred to as an “established” patient making them eligible for referral to dental and behavioral health services. Dental, behavioral health, pharmacy and laboratory services under the sliding fee scale program are only available to “established” patients.
Patients who see a non CHCY practitioner for primary care are eligible to receive reproductive health services.
Primary Medical Care
Outpatient Medical Care is available, including the following:
- Preventative Care including health screenings, e.g. well child, well adult;
- Non-emergent medical treatment of acute and chronic illnesses, including minor surgical procedures and colposcopy (limited);
- Diagnostic testing (by referral);
- Pharmacy services (by referral);
- After hours coverage;
- Hospital coverage (hospitalists).
