If you are concerned about paying your bill, please talk to someone at the admission desk who will work out a payment plan with you. Unless prior arrangements are made, please make payments on the day of service. You may pay by cash, check, money order, or MasterCard and Visa.
Payment for Services
CHCY accepts private insurance, AHCCCS, Medicare, as well as a sliding fee scale for eligible individuals. Co-payments and office charges are payable at time of service and can be paid with cash, personal check, debit or credit card. For questions concerning your statements, please contact our Business Office.
Private Health Insurance
CHCY accepts the majority of health insurance policies.

Health Insurances Accepted
AHCCCS: Bridgeway, Phoenix Health Plan
If you are currently with either of these plans, you must contact the plan and select one of our primary care practitioners as your primary care practitioner. If you are in the AHCCCS application process, you can receive care from the Community Health Center under the sliding fee scale. If you are denied AHCCCS coverage, bring in your denial letter to verify that you applied, so we can continue to serve you on our sliding fee scale.
Community Health Center of Yavapai is a Medicare Part B participating provider. Please bring your Medicare Part B card with you. We accept assignment and will bill Medicare for you. You will be responsible for 20% of the allowed charges after Medicare pays. If you have Medicare Part A only, you may be eligible to receive medical services on our sliding fee scale. Community Health Center of Yavapai accepts many private insurances. Please bring your ID card with you and we will bill your insurance for you. Once your insurance has paid, you will be responsible for any portion of your bill that is not covered by insurance, including co-payment and coinsurance.
Sliding Fee Scale
The Community Health Center of Yavapai provides quality health care to people regardless of their ability to pay. If you are uninsured and financially eligible, our sliding fee scale allows patients to pay at variable rates.
