The Community Health Center of Yavapai (CHCY) is a Federally Qualified Community Health Center, located in Yavapai County AZ. CHCY provides comprehensive primary health care services, i.e., medical, dental, reproductive health (family planning) and behavioral health services.
CHCY is committed to providing care within the “Medical Home” concept. CHCY becomes the patient’s Medical Home when the patient receives their primary medical care from a CHCY provider. Once a patient has established this relationship, the patient is eligible for referral for dental and behavioral health services.

The Community Health Center of Yavapai has been serving the public since 1993 and recently opened the doors to a brand new facility in Cottonwood.
Last year the Community Health Center of Yavapai served over 11,000 patients who visited our three locations over 35,000 times.
